Medical Insurance
By university rule, all international students at FIU in F-1 and J-1 status are required to have medical insurance prior to registration. Students will NOT be allowed to register for classes unless they comply with the medical insurance requirement (an "IMI" service indicator appears on student's record). Students must first document medical insurance coverage meeting the minimum requirements and clear with ISSS before they are allowed to register. The medical insurance requirement can be met in one of two ways:
- By purchasing the University-approved policy OR
- By submitting an International Student Health Insurance Compliance Form completed by your current insurance company; MUST be approved by UHS (This option available only to students in F-1 status.).
I. University-Approved Insurance
The 2008-2009 University-approved insurance
provider is UniCare Life and Health Insurance
Company. The University-approved
medical insurance policy may be purchased
by one of the following methods:

Once you have submitted the application and your payment clears, you will receive confirmation indicating that the credit card transaction was successful. PRINT THIS PAGE AND KEEP IT FOR YOUR RECORDS or to get your hold removed immediately bring a copy of this page to UHS.
Money Order or Cashier's Check: Purchase the insurance policy at University Health Services at UP or at BBC by the following steps:
(1) Complete and sign the appropriate insurance
enrollment form. Students submitting semi-annual
2 payment must enclose bill.
(2) Submit payment in the form of a Money Order or Cashier's Check payable to Unicare Life & Health Insurance Company. Personal checks are NOT accepted.

"IMI" service indicators will NOT be lifted until students comply with the FIU medical insurance requirement. There are NO EXCEPTIONS to this rule.
II. Alternate Insurance Policy
If you have your own insurance policy (or wish to purchase a policy other than the University-approved policy), you will need to have your insurance company complete an INTERNATIONAL STUDENT HEALTH INSURANCE COMPLIANCE FORM and have completed form faxed to UHS. The completed compliance form must verify the following:
that the coverage meets or exceeds the minimum FIU requirements ,
the name, address, and telephone number of a claims agent in the United States,
that you are covered under the policy, and
that your insurance has been paid through current year.
IMPORTANT NOTICE:
Please note that effective Fall 2008, the medical insurance requirements have changed. Having had an Alternate Insurance policy approved in 2007-2008 (or earlier) does NOT guarantee that the same policy will be approved for 2008-2009.
You must submit the: 2008-2009 Alternate Insurance Policy Form in order to receive clearance for the 2008-2009 academic year.
Your policy will be reviewed, and if it complies with the guidelines, you will be cleared to register. If your policy does not comply with the guidelines, you will not be cleared until you document insurance coverage which meets FIU requirements or purchase the University-approved insurance policy. General questions may be answered over the phone but alternate insurance policies cannot be evaluated without the completed Alternate Insurance Compliance Form.
If your alternate policy is approved subject to Medical Evacuation & Repatriation. The service indicator will not be released until this requirement is met. If you so choose, you may purchase coverage for Medical Evacuation & Repatriation from Hill Chesson.
NOTE: You should NOT purchase Medical Evacuation and Repatriation UNTIL you have proof that your alternate insurance policy was approved subject to it.
ATTENTION: FIU Graduate Assistants
FIU provides subsidized health insurance to all graduate assistants holding a 0.5 FTE full term appointment as a Graduate Assistant, Presidential Fellows and Dissertation Fellows. All graduate assistants will be enrolled in the new Graduate Assistant health insurance program and will be cleared of the insurance service indicator (IMI) from a list provided by the Graduate School. Graduate Assistants will pay 25% of the insurance premium through payroll deduction.

FIU Immunization Requirements
Per FIU requirements, all new degree & non-degree seeking students under the age of 40 enrolling at FIU MUST provide documentation of both Measles and Rubella immunity. FIU students MUST comply with this requirement at least one week PRIOR to registration. For more detailed information, please visit the Immunizations section of
