All international students and scholars at FIU are required to have medical insurance. The FIU-approved international student health insurance policy is available for purchase.
If you are an international student

If you plan to use your own private insurance policy, your insurance company must complete the International Student Health Insurance Compliance Form and send it directly to Health Compliance for review at least two (2) weeks prior to registration. Your policy must be reviewed and approved by Health Compliance to meet the minimum requirements established by the Florida Board of Governors (BOG) for international student health insurance before you can be cleared to register.
The form MUST verify:
- that the coverage meets or exceeds minimum requirements,
- the name, address, & telephone number of a claims agent in the U.S.,
- that you are covered under the policy, and
- that your insurance has been paid through the current year.
If your policy does not comply with the FL BOG requirements, you will NOT be cleared to register for classes. until you provide documentation of insurance coverage which meets FL Board of Governors requirements. Alternate insurance policies cannot be evaluated without the completed International Student Health Insurance Compliance Form. And a copy of your summary of benefits.
If you are an international visiting scholar
You are required to have health insurance that meets certain requirements of the United States Department of State’s Bureau of Educational and Cultural Affairs, the State of Florida, and Florida International University for you, your spouse, and all children who are listed on the DS-2019. The insurance policy must include medical evacuation and repatriation benefits in addition to major medical. FlU has approved insurance plans that meet these requirements, which J-1 Visiting Students, scholars and their dependents may purchase.

Medical insurance for dependents must be purchased prior to the issuance of the DS-2019.
If you have your own policy, you will need to complete an International Scholar Health Insurance Compliance Form and submit it with your required documentation. The policy must be approved as meeting the minimum requirements of the University before you can be cleared to receive your DS-2019. The form MUST verify:
- that the coverage meets or exceeds minimum requirements,
- the name, address, & telephone number of a claims agent in the U.S.,
- that you are covered under the policy, and
- that your insurance has been paid through the current year.
ISSS will review your policy and, if it complies with the requirements, will clear you. If your policy does not comply with the requirements, you will NOT be cleared to receive your DS-2019. Alternate insurance policies cannot be evaluated without the completed International Scholar Health Insurance Compliance Form.
Note: An insurance card is NOT valid proof of insurance. SHS staff cannot evaluate individual plans without the compliance forms. If the insurance company refuses to fill out the form, the alternate insurance policy will not be approved.
Note: A student in J-1 status is required to purchase the university-approved medical insurance policy for himself or herself and all accompanying J-2 dependents.
REGISTRATION HOLDS (also known as Service Indicators) WILL NOT BE CLEARED UNTIL YOU COMPLY WITH THE MEDICAL INSURANCE REQUIREMENT. WE CANNOT MAKE EXCEPTIONS TO THIS RULE.
